A note on Thiel's known interventions

Some of this requires a prescription. Most of it does not.

Peter Thiel is publicly known to take human growth hormone (hGH), metformin, and testosterone replacement — all prescription medications that should only be used under direct medical supervision. This page does not recommend those interventions. What it does recommend is the underlying philosophy: aggressive biomarker tracking, proactive blood sugar management, and optimising the body's own hormonal pathways through diet, exercise, and evidence-backed supplements. These OTC equivalents are legal, accessible, and well-supported by research.

The Thiel Philosophy
📊
Comprehensive blood panels every 3–6 months
Not just the standard cholesterol and glucose panel — Thiel's framework tracks HbA1c (3-month blood sugar average), homocysteine (cardiovascular and cognitive risk), IGF-1 (growth hormone proxy), testosterone (free and total), Vitamin D (25(OH)D), and high-sensitivity CRP (systemic inflammation). These markers catch trajectories years before disease develops. A doctor will order these if you ask.

Treat health like a portfolio — data-driven, proactive
Thiel's core insight is that waiting until symptoms appear means intervening after 10–30 years of silent progression. Every dollar spent on prevention (bloodwork, sleep, exercise, targeted supplementation) has a return profile that dwarfs late-stage treatment costs. The analogy he uses: you would not run a company by waiting for the quarterly P&L to turn negative before looking at your numbers.

Home Blood Test Kit For tracking basic markers between doctor visits
At-home finger-prick kits for HbA1c, cholesterol, and CRP give you a data point between annual physicals. Useful for seeing how diet or supplement changes move your numbers in real time rather than once a year.
Continuous Glucose Monitor (CGM) Thiel's approach: manage blood sugar proactively, not after diabetes develops
A CGM worn for 2–4 weeks shows exactly how your blood glucose responds to specific meals, sleep patterns, and stress — information that a single fasting glucose test cannot provide. Most non-diabetic users discover that 2–3 specific foods spike them far more than expected. That data alone changes eating behaviour permanently.
Blood Sugar Management
🥗
Low-glycaemic diet with a fasting window
Delaying the first meal 12–16 hours from the last meal of the previous day keeps insulin low for an extended period, improving insulin sensitivity and reducing fasting glucose over weeks. Pairing this with a low-glycaemic food pattern — prioritising fibre, protein, and fat over refined carbohydrate at each meal — flattens the glucose response across the day. Thiel's metformin achieves a similar effect mechanistically (AMPK activation) but diet achieves it upstream.

Exercise before or after meals to blunt glucose spikes
A 10-minute walk after a large carbohydrate meal reduces the post-prandial glucose spike by 20–30% by using skeletal muscle as a glucose sink — no insulin required. Training in a fasted state depletes muscle glycogen further, improving the capacity to absorb glucose from the next meal. This is one of the most cost-free blood sugar interventions available.

Berberine 500mg The most evidence-backed OTC equivalent to metformin
Berberine activates AMPK — the same cellular energy sensor that metformin targets. Multiple head-to-head RCTs in type 2 diabetics show comparable HbA1c reduction to metformin at 1500mg/day (500mg three times daily with meals). For a healthy person using it for longevity, the effect is a sustained blunting of insulin resistance. Take with meals to avoid GI discomfort.
Ceylon Cinnamon (true cinnamon, not Cassia) Blunts post-meal glucose spikes when taken with meals
Ceylon cinnamon (Cinnamomum verum) contains compounds that slow gastric emptying and improve glucose uptake in muscle cells. 1–3g with meals reduces post-prandial glucose in multiple controlled studies. Ceylon specifically — not Cassia (the common supermarket variety) which contains coumarin at levels that are hepatotoxic at supplementation doses.
Natural GH Optimisation
⏱️
Extended fasting — the #1 natural growth hormone trigger
A 24-hour fast raises serum growth hormone approximately 5-fold. The mechanism is partly the drop in blood glucose and insulin (which suppress GH), and partly the direct fasting signal on the hypothalamus. Shorter fasting windows (16-hour) produce a smaller but still meaningful effect. Thiel's exogenous hGH bypasses this entirely — but fasting achieves the same axis activation through natural means.

Heavy resistance training and HIIT — GH spikes for 24 hours post-exercise
High-intensity exercise, particularly heavy compound lifts and sprint intervals, produces the largest exercise-induced GH pulse. The spike peaks in the hour after training and the elevated baseline persists for roughly 24 hours. The combination of compound resistance training and a weekly HIIT session gives you multiple GH pulses per week without any pharmaceutical intervention.

🛌
Sleep onset — the largest daily GH pulse happens 90 minutes after falling asleep
The pituitary releases its largest daily GH bolus approximately 90 minutes after sleep onset, exclusively during deep (slow-wave) sleep. This is the natural pulse that exogenous hGH supplementation seeks to replicate. Every factor that degrades sleep quality — late eating, alcohol, blue light, high pre-sleep cortisol — directly reduces GH output. Protecting sleep is protecting GH.

Zinc + Magnesium (ZMA stack) The most evidence-backed combination for natural GH and testosterone support
Zinc is rate-limiting for testosterone synthesis — even marginal deficiency suppresses production. Magnesium improves deep sleep quality, which is the GH pulse window. The ZMA combination (zinc monomethionine + magnesium aspartate + B6) taken before bed has shown GH and testosterone increases of 30–40% in deficient individuals in multiple studies.
GABA 750mg Natural GH secretagogue — taken before sleep or training
Oral GABA at 3g has raised GH by 400% in exercise studies; 750mg is a gentler starting dose with lower risk of morning grogginess. The mechanism is direct action on GH-releasing neurons in the hypothalamus. Best taken 30–60 minutes before sleep or a training session. Not a sedative at this dose — it supports deep sleep without suppressing REM.
Longevity Supplements
NMN 500mg NAD+ precursor — the closest OTC analogue to metformin in the longevity pathway
NMN raises NAD+ levels, which activate sirtuins (the cellular repair proteins) and support mitochondrial function. Thiel and the broader longevity community view NAD+ restoration as one of the most compelling targets in the aging biology literature. NMN is more stable and better absorbed than its precursor NR in most formulations.
Resveratrol Trans Form SIRT1 activator — synergistic with NMN
Trans-resveratrol activates SIRT1, the sirtuin that NMN's elevated NAD+ enables. David Sinclair's Harvard research (the primary source for Thiel's longevity stack) shows the two are synergistic — NMN without resveratrol is substrate without enzyme. Use the trans- form specifically; cis-resveratrol is biologically inactive. Take with a fat source to improve absorption.
Omega-3 Fish Oil — high dose (2g EPA/DHA) The most replicated supplement for all-cause mortality reduction
EPA and DHA reduce triglycerides, lower systemic inflammation (CRP), and are structural components of neuronal membranes. Multiple meta-analyses confirm cardiovascular mortality reduction at doses above 1g EPA+DHA daily. The "fish oil" weight on the label is irrelevant — look for the combined EPA+DHA figure and aim for 2g.
Vitamin D3 2000 IU — Briyosis
Vitamin D deficiency is independently associated with increased all-cause mortality across multiple large population studies. The majority of Indians are deficient despite year-round sun exposure due to skin melanin, air pollution, and indoor work patterns. 2000 IU is a safe daily maintenance dose; get your 25(OH)D tested to confirm you are above 40 ng/mL.
Exercise for Longevity
💪
Strength training — sarcopenia is a primary driver of aging
Loss of muscle mass (sarcopenia) accelerates after 40 at roughly 1% per year without intervention and is independently predictive of mortality, metabolic disease, fall risk, and disability. Thiel's framework treats muscle preservation as a longevity target, not an aesthetic one. Resistance training 2–3x per week with progressive overload is the only intervention that reliably reverses sarcopenia at any age.

150+ minutes moderate exercise per week — the minimum effective dose
The longevity literature consistently shows the largest mortality benefit in the transition from sedentary to minimally active — roughly 150 minutes of moderate activity per week. Thiel approaches exercise as medicine: minimum effective dose, rigorously applied. Zone 2 cardio (conversational pace) for mitochondrial health, heavy resistance for muscle mass. Not more than needed, never less.
Free
Resistance Bands Set (heavy + medium + light) Minimal equipment for compound movements anywhere
Heavy resistance bands (40–50kg equivalent) enable compound lower body work without a barbell. Banded squats, hip hinges, rows, and presses provide the progressive resistance stimulus for muscle preservation. Removes friction from the consistency equation — the session that actually happens beats the perfect session that doesn't.
CoQ10 Ubiquinol 100mg Mitochondrial energy production — synergistic with exercise and NMN
CoQ10 is the electron carrier in the mitochondrial ATP synthesis chain. Endogenous levels decline with age and are further depleted by statin use. The Ubiquinol form (the reduced, active form) is 2–4x more bioavailable than the cheaper Ubiquinone form. Taken with the largest meal of the day for fat-assisted absorption. Synergistic with NMN at the mitochondrial level.
